About the role
Key responsibilities & impact- Ensure initial coordination of information among clients, family members, physicians, referral sources, facility staff, agency staff, and others involved in continuity of care
- Inform referral sources, clients, family members, and physicians about response times and follow-up plans
- Inform clinical office staff of potential admissions
- Accurately document information in clients’ medical records
- Input patient information into the EMR and update it with the correct level of care
- Complete and copy paperwork required to prepare patient files for admission
- Complete referral intake forms and copy, scan, and upload referral information for clinical review and admissions clinicians
- Establish, maintain, and close patient EMR and paper charts according to regulatory and office guidelines
- Mail, fax, or email required documentation
- Verify insurance coverage and collaborate to obtain needed information
- Create morning and evening census reports
Requirements
What you’ll need- Associate degree is preferred and equivalent with successfully completed course work or equivalent experience in medical terminology and/or working in healthcare environments
- Minimum of two years’ experience working in hospice REQUIRED
- Experience dealing with Medicare, Medicaid, and commercial payors preferred
- Customer service focus, including ability to set callers at ease and encourage open dialogue about client needs
- Ability to work with team members across functional areas affecting client intake/admission
- Ability to maintain composure in stressful and challenging situations
- Prior use of Electronic Medical Records (EMR) preferred
- Must be a licensed driver with an insured automobile meeting state and/or organization requirements and in good working order
- Availability Monday-Friday, 3PM-11PM
- On-call availability Monday-Friday, 11PM-8AM, plus one shift every 12th weekend
- Territory coverage in Barrington, Deerfield, Crystal Lake, or Palos Heights
