About the role

Key responsibilities & impact
  • Function as a medical case manager for assigned individuals served in the Family Model Program
  • Monitor individuals' service plans in accordance with regulatory guidelines
  • Coordinate and arrange appointments, meetings, and other administrative matters
  • Provide supportive assistance to individuals served and their families
  • Work with other agencies on behalf of assigned individuals
  • Assist the Director in interviewing prospective individuals served and their families
  • Use computerized systems to schedule appointments and maintain patient records
  • Perform filing, data entry, and office management
  • Use medical terminology accurately in communication and documentation
  • Support community acceptance through community activities and liaison efforts with agencies and entities
  • Perform additional duties assigned by the Director or required by the needs of individuals served
  • Maintain regular communication with leadership and team members
  • Respond promptly during business hours while working from home

Requirements

What you’ll need
  • Must live within a one hour drive of Hermitage, TN
  • DDA/ECF/CLS experience
  • Must have a 27" or larger monitor, keyboard, and mouse for use with the company-issued laptop, unless equipment is loaned by the company
  • High-speed internet
  • Ability to prepare necessary reports and records of individuals' program progress accurately and completely
  • Knowledge of practices and techniques of programming for individuals with intellectual disabilities
  • Ability to exercise judgment in interpreting and implementing programs
  • Ability to implement proper practices and techniques of supervision
  • Effective verbal and written communication with staff, individuals' families, individuals, and outside service providers
  • Flexibility and organization
  • Strong administrative skills and attention to detail
  • Ability to multitask and prioritize tasks effectively

  • Ability to work without close supervision
  • Ability to interact with a wide range of people
  • Computer skills including MS Word, Excel, Excel Spreadsheets, Outlook, Google Docs, and Windows

  • Timely responsiveness during business hours
