About the role
Key responsibilities & impact- Manage a designated client caseload
- Travel statewide to assess and evaluate nursing-home residents eligible for transitions back to the community
- Complete MFP/DSS paperwork
- Provide clinical expertise to team members
- Conduct recovery plan reviews, service authorizations, and care coordination for assigned clients
- Maintain current knowledge of behavioral health benefits, coverage limitations, exclusions, policies, and procedures
- Supervise collection and analysis of service delivery and outcome information
- Provide accurate information about waiver benefits and coverage to clients, providers, and stakeholders
- Customize services with clients and providers to meet client needs within program scope
- Develop and implement systematic service improvements
- Maintain daily logs and prepare weekly trend reports
- Coordinate information and presentations for providers, government agencies, community groups, and other stakeholders
- Link clients and families with primary care, behavioral health, social service, and provider agencies
- Collaborate on diagnostic screenings, referrals, assessments, treatment planning, and aftercare planning
- Support clients transitioning into or out of the community
- Monitor treatment and service plan effectiveness and recommend modifications
- Attend case conferences, interagency meetings, and provider treatment planning meetings
- Receive regular supervision from the Lead Intensive Care Manager
- Provide clinical expertise to WISE program staff
- Participate in professional development and required training
- Maintain confidentiality of protected health information and comply with HIPAA policies
- Demonstrate ethical behavior and cultural sensitivity
- Perform other duties supporting business operations
Requirements
What you’ll need- Must have a Master's Degree in a related field (social work, psychology, etc.)
- LCSW, LPC or LMFT licensure, or licensed eligibility
- Minimum 3 years of behavioral health clinical experience
- Experience delivering community-based services
- Compliance with current State of Connecticut and federal health and vaccine requirements
- General knowledge of behavioral health best practices, especially with special needs or priority populations and public sector systems
- Advanced-level knowledge of service delivery systems, provider networks, client services policies, client rights and responsibilities, and organizational operating practices
- Excellent clinical skills and sophisticated understanding of clients' overall needs from a strengths-based perspective
- Demonstrated knowledge of clinical treatment and case management
- Ability to propose and implement creative solutions to client problems
- Excellent clinical, written and oral communication skills
- Ability to multitask and meet assigned deadlines
- Must have reliable transportation
- Must travel in southwestern Connecticut at least 4 days per week
- No relocation available
