About the role

Key responsibilities & impact
  • Coordinate and implement evidence-based community health programming supporting needs identified at designated hospital sites
  • Prioritize collaboration with community partners
  • Coordinate program tactics including attending meetings, ordering supplies, gathering data, coordinating events and screenings, and supporting community partners
  • Collect monitoring and outcome data for community health programs
  • Participate in site Community Health Council meetings and other meetings promoting system alignment with community health strategies
  • Collaborate with the director on data collection activities related to the Community Health Assessment
  • Educate associates, management and hospital governance on community health issues and planned hospital interventions
  • Implement activities ensuring accurate collection and summary of community benefit data
  • Facilitate reporting of community benefit activity for designated hospital sites
  • Maintain relationships with community leaders, faith groups, community-based organizations, political/government officials, healthcare organizations and other hospitals
  • Represent designated hospital sites in local coalitions addressing community health needs
  • Communicate CHNA findings to community groups and solicit involvement in addressing identified needs

Requirements

What you’ll need
  • High School Diploma or GED
  • 2 years of experience addressing and improving the social drivers of health, community health strategy, population health initiatives, or related areas
  • Strong analytical and writing skills
  • Knowledge of current and emerging best practices in large health systems addressing non-clinical drivers of health
  • Ability to coordinate community health programs, partnerships, meetings, events and related implementation activities across internal and external stakeholders
  • Ability to collect, monitor and summarize program, community health assessment and community benefit data to support reporting, compliance and continuous improvement
  • Computer literacy in Microsoft programs including Excel, Publisher, Word and Access
  • Ability to travel within the metro community
  • Frequent need to attend off-site meetings
  • Able to work on evenings or week-ends in community
  • Bachelor's degree in public health, epidemiology, social work, community development, public policy or a related field addressing the social drivers of health (preferred)
