About the role
Key responsibilities & impact- Design and direct assigned Social Health Operations areas, including referral operations support, CHW workflow infrastructure, operational education, community resource connectivity, and implementation support
- Select, coach, and develop team members; set objectives; manage performance; support succession planning; and create feedback, recognition, and performance-improvement processes
- Serve as enterprise operational lead for Social Health initiatives across operations, interventions, CHW programs, virtual care, clinical screening, and other stakeholders
- Translate enterprise Social Health strategy into operating plans, standards, workflows, playbooks, and adoption plans
- Lead governance and performance routines, including issue escalation, decision tracking, implementation readiness, standard-work maintenance, and progress reporting
- Develop enterprise reporting, executive updates, governance materials, and performance reviews
- Partner with IT, Clinical Informatics, Analytics, Community Health, Care Management, state division leaders, operations, training teams, and vendors to implement and optimize workflows and referral tools
- Coordinate enterprise Social Drivers of Health screening and referral initiatives
- Direct improvement initiatives to increase referral completion, strengthen closed-loop performance, improve resource visibility, reduce workflow variation, and address identified barriers
- Oversee education, onboarding, super-user structures, communications, and change-management resources
- Lead enterprise communication and stakeholder engagement efforts
- Develop or recommend operating and capital budgets and control expenditures
- Ensure adherence to the organization’s Code of Ethical Conduct and applicable policies, regulations, and laws
Requirements
What you’ll need- Bachelor’s degree in healthcare administration, public health, social work, business administration, health services, or related field
- Seven or more years of experience in healthcare operations, population health, social care, community health, care management, health equity, or related field
- Three or more years of management-level experience
- Experience with complex operational initiatives
- Knowledge of social drivers of health, referral operations, CHW workflow infrastructure, community resource networks, healthcare operations, performance improvement, change management, stakeholder engagement, data-informed decision making, team leadership, and project or program management
- Ability to translate strategy into operational plans
- Ability to lead through influence and manage competing priorities
- Ability to drive consistent execution across a matrixed environment
- Master’s degree preferred
- Lean, Six Sigma, PMP, change management, or similar certification preferred
- Experience leading Social Health, SDOH, community health, CHW, population health, care management, or health equity operations in a large integrated health system preferred
- Experience implementing enterprise workflows, referral platforms, resource navigation tools, or closed-loop referral processes preferred
- Advanced ability to build operational infrastructure, lead managers or senior professionals, run governance routines, manage stakeholder relationships, interpret performance data, and guide medium- to long-term functional plans
- Periodic travel to system, division, market, and community locations may be required
