About the role

Key responsibilities & impact
  • Provide competent home health aide care to assigned patients
  • Assist patients with activities of daily living according to the HHA assignment record
  • Promote patients’ functional status goals while maintaining safety
  • Observe and report changes in patient condition to the case manager or RN/Supervisor
  • Provide specialized care to terminally ill patients in accordance with hospice philosophy
  • Perform personal care, feeding, laundry, light cleaning, and washing
  • Safely use patient-care equipment
  • Take temperatures, pulse, respiration, and blood pressure as directed
  • Provide skin care and reposition patients to prevent breakdown
  • Change patient bed linens and clothing as requested
  • Work independently and maintain a consistent, professional schedule
  • Cooperate with team members and remain calm in patients’ homes, including around death and dying
  • Participate in case conferencing and IDGs to support patient outcomes
  • Prepare timely documentation in accordance with agency, state, federal, and accrediting-organization standards
  • Maintain patient confidentiality and comply with HIPAA/privacy requirements
  • Participate in CQI, quality-improvement projects, problem solving, and patient-satisfaction efforts
  • Maintain required in-service, hospice education, OSHA, and state training hours
  • Cooperate with required supervisory visits
  • Provide services in accordance with regulatory rules and standards
  • Support department, service-office, team, company, and organizational goals
  • Identify developmental needs and maintain home health aide skills

Requirements

What you’ll need
  • High school education or equivalent
  • One year of nurse’s aide experience
  • RN nursing students who meet state requirements
  • Certified as a nursing assistant in the state in which the teammate works
  • Valid driver’s license issued by the Division of Motor Vehicles
  • Written and verbal competency in English
  • Ability to recognize the needs and behaviors of patients across a variety of age groups
  • Understanding of child growth and development, adolescent behavior, and signs of normal aging
  • Ability to travel throughout service office boundaries in all weather and road conditions
  • Ability to assist with ambulation, turning, and transferring patients up to 200 pounds
  • Ability to lift up to 35 pounds without assistance; use patient-handling equipment with another associate for heavier or non-assisting patients
  • Ability to make visits to private residences that may not be accessible to people with disabilities
