About the role
Key responsibilities & impact- Assume responsibility for caseloads consisting of patients in specialty areas
- Conduct initial assessments and obtain thorough physical, social, and environmental data
- Establish plans of care according to physician orders and patient needs
- Provide direct patient care and adjust nursing care processes as needed
- Identify problems, modify nursing care plans, and evaluate outcomes
- Advise and consult with families and other personnel
- Assess patient responses to treatments and teaching and apply interventions
- Update and revise plans of care
- Complete visit assessments, OASIS, 485, and verbal-order documentation accurately and promptly
- Communicate with physicians to confirm and update patient plans of care
- Provide status updates and maintain communication with physicians and healthcare team members
- Facilitate discharge planning and continuity of care after discharge
- Participate in chart audits, continuous quality improvement, and process-improvement activities
- Maintain accurate clinical and patient records according to agency, state, and federal guidelines
- Provide age-appropriate care based on patient status and developmental needs
Requirements
What you’ll need- Associate's Degree in Nursing
- Registered Nurse license issued by the state in which the team member practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) must be obtained within 6 months unless determined not required
- Valid driver's license issued by the Division of Motor Vehicles, or driver's license issued by the state of Illinois
- Typically requires 3 years of experience in clinical nursing
- Excellent communication skills
- Strong interpersonal, organizational, analytical, and problem-solving skills
- Ability to work with physicians and other professionals in a direct and positive manner
- Proficiency in clinical skills and ability to make sound judgments
- Ability to operate fax, computer, copier, and telephone systems
- Must possess and provide proof of a valid driver's license and a registered, functional car with adequate insurance coverage
- Ability to meet physical requirements including lifting up to 50 lbs from floor to waist, lifting up to 20 lbs from waist to overhead, carrying up to 40 lbs at waist height, pushing/pulling 30 lbs of force, and performing a sliding transfer of 150 lbs with a second person present
- Ability to work safely with patients of various sizes, weights, medical conditions, and physical limitations
- Ability to tolerate temperature changes and exposure to chemical, hazardous waste, and communicable disease
