About the role

Key responsibilities & impact
  • Lead inpatient care management teams at Atrium Health Union, Union West, and Anson
  • Oversee daily operations of care management and social work programs
  • Provide direct leadership, staff development, orientation, competency assessment, and performance management
  • Approve and monitor staff schedules, paid time off, and timecards
  • Monitor staff proficiency and implement action plans to address deficiencies and improve efficiency
  • Facilitate team building, conflict resolution, and performance improvement activities
  • Establish performance measures, targets, and benchmarks
  • Collect, analyze, and report data on care processes and performance variations
  • Identify root causes of performance gaps and develop documented improvement plans
  • Review site successes and improvement opportunities with leadership and stakeholders
  • Serve as an internal consultant and expert resource for challenging cases and regulatory requirements
  • Collaborate with physicians, physician advisors, nurses, hospital leaders, post-acute partners, patients, and families to support care progression and discharge planning
  • Provide education on care coordination, alternative payment models, regulatory standards, and performance improvement
  • Participate in utilization management committees and multidisciplinary cross-functional initiatives
  • Manage and recommend operating and capital budgets and control expenditures
  • Ensure compliance with CMS, OSHA, accrediting organizations, state and federal guidelines, and ethical standards
  • Perform human resources responsibilities including interviewing, selection, promotions, evaluations, compensation changes, corrective actions, terminations, and employee morale

Requirements

What you’ll need
  • Bachelor's Degree in Nursing or Master's Degree in Social Work or related field
  • If BSN, Registered Nurse license issued by the state in which the team member practices is required
  • Social worker license/certification issued by the state in which the team member practices is preferred or required by the state
  • Obtain ACM or SW (ACM), CCM, RN-BC, or C-SWHC certification within 1 year
  • 3 years of experience in a relevant clinical setting
  • Master's in Social Work preferred
  • Ability to manage care management and social work programs
  • Knowledge of CMS, OSHA, accrediting organization, regulatory, utilization management, and care management practice requirements
