About the role
Key responsibilities & impact- Assess, diagnose, and determine or alter treatment and management plans
- Order, perform, and interpret appropriate diagnostic studies
- Prescribe pharmacologic and non-pharmacologic interventions and therapies
- Manage conditions using clinical indication, evidence-based care, cost effectiveness, and risk-benefit assessment
- Provide health promotion, disease prevention, and disease management counseling and education
- Manage patients as part of an interdisciplinary team and within scope of practice
- Seek consultation or referrals and escalate emergent or specialized care when necessary
- Maintain accurate, complete, concise, and timely electronic medical record documentation
- Submit professional services consistent with compliant coding and billing practices
- Facilitate coordinated care and communication among healthcare team members and health or community agencies
- Perform office or hospital procedures according to specialty practice, competency, and granted privileges
- Participate in quality, safety, peer review, performance activities, meetings, committees, and workgroups
- Participate in education and onboarding of new team members, students, and healthcare professionals
- Maintain and develop clinical and professional skills
- Maintain productivity, access, face-to-face time, and quality metrics for safe and timely patient care
- Provide care appropriate to patients’ age-specific needs
Requirements
What you’ll need- Active RN and APRN/APRN-FPA/APNP or other APRN license(s) in state(s) of practice
- Active national board certification in area of clinical practice and populations served
- Active DEA registration prior to hire
- For Illinois practice, active Illinois Controlled Substance License prior to hire
- Active BLS and/or ACLS, PALS, NRP as required by clinical practice prior to or within 6 months of hire
- Master’s Degree in Nursing
- Demonstrated high level of clinical proficiency and excellent decision-making skills
- Ability to work independently and as an effective member of a health care team
- Ability to adapt to evolving technology and proficiency with the electronic medical record
- Excellent communication and collaboration skills
- Proven organizational skills and ability to prioritize effectively
- Ability to stand, walk, bend, stoop, and twist continuously throughout the workday
- Functional speech, vision, touch, and hearing
- Ability to lift up to 50 lbs from floor to waist
- Ability to lift up to 20 lbs over the head
- Ability to carry up to 40 lbs a reasonable distance
- Ability to operate all equipment necessary to perform the job
