About the role
Key responsibilities & impact- Provide video consultation and triage to patients, families, and significant others according to the Home Health Virtual Program plan of care
- Provide ongoing care via video according to established plans of care
- Determine patient-care priorities based on essential needs and available resources
- Recognize changes in patient status and intervene appropriately
- Coordinate physician contacts, consultations with the home health care team, visit strings, physician-order changes, and care-plan goals
- Assess patient, family, and caregiver training needs and provide teaching and counseling via video consultation and triage
- Collect and maintain clinical and patient records according to agency, state, and federal guidelines
- Maintain primary responsibility for video documentation, data review, quality, and follow-up of patient information
- Assist with reports and statistics related to key clinical performance measures
- Develop, implement, and evaluate professional standards to ensure quality of care
Requirements
What you’ll need- Home Health experience required
- Physical Therapist license issued by the state in which the teammate practices; licensure in at least one supported state is required prior to hire if supporting multiple states
- Bachelor of Science, Masters, or Doctorate Degree in Physical Therapy
- Typically requires 5 years of experience in combination of rehab, home care and triage settings
- Strong communication skills and attention to detail
- Ability to coordinate and manage complex care for patients over video
- Motivated and self-directed with above average interpersonal skills
- Ability to work positively with management, physicians, and other professionals
- Ability to work in a fast-paced environment and under time constraints
- Advanced computer skills using Microsoft Office applications
- Overall proficiency and comfort level with computer systems
- Knowledge and skills necessary to provide age-appropriate care
- Ability to assess patient data and interpret information relative to age-specific needs
