About the role
Key responsibilities & impact- Provide care following the National Standards for Diabetes Self-Management Education and Support as defined by ADA or ADCES
- Assess patient learning needs, goals, and barriers to care and tailor educational interventions
- Lead individual and group education sessions on diabetes management, including medication, nutrition, exercise, and blood glucose monitoring
- Provide formal inpatient and/or outpatient diabetes management instruction
- Provide medical nutrition therapy for patients with diabetes or other referring diagnoses when a Registered Dietitian
- Provide ongoing diabetes-management support and follow-up
- Maintain knowledge of diabetes-management technology and manufacturer protocols
- Collaborate with the healthcare team to develop and implement comprehensive care plans
- Adjust diabetes medications per protocol under provider order, proven competencies, and departmental policy
- Stay current on diabetes-care developments and ensure compliance with ADA standards
- Document and charge for services in accordance with CMS and payer requirements for DSMT and MNT services
- Participate in community events and coordinate support meetings
- Participate in development and evaluation of patient education programs and materials
- Act as a health resource for healthcare providers, patients, and the community
Requirements
What you’ll need- Current license or certification to practice as a Registered Nurse (RN) or Registered Dietitian in the applicable state
- Degree in Nursing, Dietetics, or another related discipline meeting CDCES certification eligibility requirements
- Master's degree in Dietetics required if Dietitian license was obtained after 01/01/2024
- Certified Diabetes Care and Education Specialist (CDCES) preferred; if not certified, certification must be obtained within two years, not to exceed three years
- Two years of patient-oriented educational experience preferred, with broad diabetes knowledge across age-appropriate populations and co-existing health conditions
- Strong oral and written communication skills
- Organizational and effective time management skills
- Flexibility to adapt to changes in daily routine
- Public speaking skills or potential to develop them
- Self-directed
- If not yet CDCES-certified, completion of required diabetes-related continuing education hours annually per ADA/ADCES requirements
- Knowledge and skills appropriate to the age of patients served
- Knowledge of growth and development principles across the life span
- Ability to work occasional irregular hours
- BSN preferred for nursing degree
