About the role
Key responsibilities & impact- Oversee pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens
- Educate patients and caregivers about medication therapy, adverse effects, compliance, appropriate use, handling, and administration
- Design, recommend, monitor, and evaluate patient-specific pharmacotherapeutic regimens
- Implement and document medication therapy recommendations
- Review and accurately interpret medication orders and prescriptions
- Participate in management of medical emergencies when applicable
- Support medication safety and quality improvement activities and report events through appropriate channels
- Perform quality assurance and performance improvement of the medication use system
- Collaborate with the healthcare team on formulary adherence and the medication-use process
- Assist with acquisition of non-formulary, compassionate use, and investigational medications
- Practice in compliance with organizational policies and local, state, and federal regulations
- Prepare, dispense, and oversee proper storage and handling of medications
- Serve as preceptor for pharmacy students, residents, and healthcare learners
- Mentor, orient, train, and assist with performance evaluations of pharmacy teammates, residents, and students
- Direct and check the accuracy of pharmacy support team members’ work to ensure safe and efficient workflows, service standards, and regulatory compliance
- Provide age-appropriate patient care based on assessment of patient status and developmental needs
Requirements
What you’ll need- Licensed in the state(s) of practice or eligible for reciprocity required
- Immunization and non-vaccine injectable certification required based on area of practice
- Credentialing and privileging required based on area of practice
- Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP
- No experience required
- Maintains skills in area of practice
- Strong computer skills
- Excellent communication and organization skills
- Excellent presentation, facilitation, and training development skills
- Decision making, team building, customer service, and conflict resolution skills
- Life support training courses may be required dependent on department discretion
- Ability to walk considerably, stand, push a mobile cart weighing up to 50 pounds, and lift 35 pounds without assistance
- Ability to frequently stretch, bend, squat, or kneel
- Ability to use appropriate personal protective equipment and take precautions around hazardous agents and needle sticks
- Ability to perform writing, typing, and data entry into computers and automated dispensing machines
- Must have use of both hands, clear vision, and ability to read small font
- Functional speech, vision, and hearing
- Ability to operate all equipment necessary to perform the job
