About the role
Key responsibilities & impact- Assess, diagnose, and determine or alter treatment and management plans through virtual health modalities
- Order, perform, and interpret appropriate diagnostic studies
- Prescribe pharmacologic and non-pharmacologic interventions and therapies
- Manage conditions using clinical indications, evidence-based care, cost effectiveness, and risk-benefit assessment
- Leverage digital diagnostics and remote physiologic monitoring when indicated
- Provide health promotion, disease prevention, and disease management counseling and education to patients and families
- Manage patients within an interdisciplinary team and scope of practice
- Consult or refer to physicians and other healthcare team members as appropriate
- Escalate patients to emergent or specialized care when necessary
- Maintain accurate, complete, concise, and timely electronic medical record documentation
- Submit professional services using compliant coding and billing practices
- Facilitate coordinated care and communication among healthcare teams and health or community agencies
- Participate in quality, safety, peer review, performance activities, meetings, committees, and workgroups
- Participate in education and onboarding of new team members, students, and healthcare professionals
- Maintain and develop clinical and professional skills
- Maintain productivity, access, face-to-face time, and quality metrics for safe and timely patient care
- Work primarily 10-hour shifts, typically 1:00 PM to 11:00 PM, with Tuesdays off
- Provide occasional weekend and on-call coverage
Requirements
What you’ll need- Active RN and applicable APRN/NP/APRN-FPA/APNP license(s) in state(s) of practice for APRN applicants
- Active national board certification as Nurse Practitioner in the relevant clinical practice and populations served: Family, Adult, and Adult-Gero
- Active PA license(s) in state(s) of practice for Physician Assistant applicants
- Active PA national board certification from the National Commission on Certification of Physician Assistants
- Active DEA registration as applicable for state of practice
- Active Illinois Controlled Substance License if practicing in Illinois
- Master's or Doctorate's degree from an accredited Nursing Program for APRN applicants
- Bachelor's, Master's, or Doctorate's degree from an accredited PA Program for PA applicants
- Typically 3 years of experience
- High level of clinical proficiency and excellent decision-making skills
- Ability to work independently and as an effective member of a health care team
- Ability to adapt to evolving technology and proficiency with the electronic medical record
- Excellent communication and collaboration skills
- Proven organizational skills and ability to prioritize effectively
- Ability to provide age-appropriate care and assess growth and development across the life span
- Ability to stand, walk, bend, stoop, and twist continuously throughout the workday
- Functional speech, vision, touch, and hearing
- Ability to operate all equipment necessary to perform the job
