About the role

Key responsibilities & impact
  • Provide telephone-based health coaching, quality improvement, and care coordination
  • Educate patients and provide self-management support tailored to their learning style

  • Collaborate with primary care physicians and care teams to identify high-risk patients and design care-plan interventions
  • Facilitate complex care rounds with the interdisciplinary care team
  • Support population health care management initiatives
  • Measure, improve, and maintain clinical, financial, and functional quality outcomes
  • Develop and execute care plans to reduce unnecessary hospital and specialist utilization
  • Provide clinical oversight to non-licensed support staff and delegate supportive tasks
  • Use the Aledade App and real-time data to manage high-risk patients and intervene with high utilizers

Requirements

What you’ll need
  • Must hold an active, unencumbered Michigan license as either a Registered Nurse (RN) or a fully licensed Master Social Worker (LMSW)
  • Resident of Michigan
  • 3-5 years of direct healthcare experience, preferably in home health, ambulatory care, community public health, case management, or care coordination across multiple settings with multiple providers
  • Familiarity with the healthcare community being served or commitment to learn through networking and community assessment
  • Population health and/or telephonic/remote managed care experience
  • Understanding of quality metrics
  • Knowledge and experience activating patients and teaching self-management skills
  • Experience working with vulnerable populations across the lifespan
  • Competent documenting in electronic health records
  • Ability to travel up to estimated 10% as needed to office practices and staff retreats within Michigan
  • Sedentary work on telephone and computer with repetitive movements
  • Finger dexterity
  • Proficient communicative, auditory, and visual skills
