About the role

Key responsibilities & impact
  • Devise personalised support plans for older people
  • Provide access to ALONE’s services and coordinate access to other community services
  • Advocate for older people within health, social care, and welfare services
  • Establish and nurture referral relationships with statutory and non-statutory agencies
  • Develop and maintain referral pathways for ALONE’s Support Coordination services
  • Adhere to ALONE’s quality management systems and policies
  • Monitor and manage KPIs and work toward SMART targets
  • Deliver timely reports on service developments, challenges, and achievements
  • Manage the distribution and installation of technology supporting older people’s wellbeing and safety
  • Maintain records in ALONE’s CRM database

Requirements

What you’ll need
  • Demonstrated passion for improving the lives of older people and commitment to ALONE’s mission
  • Problem-solving skills focused on needs assessment and resource allocation
  • Excellent written and verbal communication abilities
  • Partnership-building and negotiation skills with diverse stakeholders
  • Proficiency in technology and data management systems
  • Good organisational and multitasking skills
  • Solution-focused approach to complex challenges
  • Keen attention to detail
  • Enthusiasm for setting and achieving SMART targets
  • Self-starter able to work well with stakeholders
  • Full clean driving licence
  • Own transport/car
  • Willingness to work 35 hours per week, 5 over 7 days
  • Field-based work and travel within the community
