About the role
Key responsibilities & impact- Devise personalised support plans to address challenges and improve older people’s quality of life
- Provide access to ALONE’s services and coordinate access to other community services
- Advocate for older people within health, social care and welfare services
- Establish and maintain referral relationships with statutory and non-statutory agencies
- Develop and maintain referral pathways for ALONE’s Support Coordination services
- Adhere to ALONE’s quality management systems and policies
- Monitor and manage KPIs and SMART targets
- Deliver timely reports on service developments, challenges and achievements
- Manage distribution and installation of technology supporting older people’s wellbeing and safety
- Maintain accurate records in ALONE’s CRM database
Requirements
What you’ll need- Demonstrated passion for improving the lives of older people and commitment to ALONE’s mission
- Problem-solving skills focused on needs assessment and resource allocation
- Excellent written and verbal communication abilities
- Ability to foster partnerships and negotiate effectively with diverse stakeholders
- Proficiency in technology and data management systems
- Good organisational and multitasking skills
- Solution-focused approach to complex challenges
- Keen attention to detail
- Enthusiasm for setting and achieving SMART targets
- Self-starter with capacity to work well with stakeholders
- Full clean driving licence and own transport are mandatory
- Field-based work and travel within the community required
- Willingness to complete ALONE’s comprehensive induction and training programme
