About the role

Key responsibilities & impact
  • Devise personalised support plans to address challenges and enhance older people’s quality of life
  • Provide access to ALONE’s services and coordinate access to other community services
  • Advocate for older people’s needs within health, social care, and welfare services
  • Establish and nurture referral relationships with statutory and non-statutory agencies
  • Develop and maintain efficient referral pathways for ALONE’s Support Coordination services
  • Adhere to ALONE’s quality management systems and policies
  • Monitor and manage KPIs and achieve SMART targets
  • Deliver timely reports on service developments, challenges, and achievements
  • Manage distribution and installation of technology supporting older people’s wellbeing and safety
  • Maintain accurate records in ALONE’s CRM database

Requirements

What you’ll need
  • Demonstrated passion for improving the lives of older people and commitment to ALONE’s mission
  • Problem-solving skills, focusing on efficient needs assessment and resource allocation
  • Excellent written and verbal communication abilities
  • Ability to foster partnerships and negotiate effectively with diverse stakeholders
  • Proficiency in technology and data management systems
  • Good organisational and multitasking skills
  • Solution-focused approach to complex challenges
  • Keen attention to detail
  • Enthusiasm for setting and achieving SMART targets
  • Self-starter with capacity to work well with stakeholders
  • Full clean driving licence and own transport are mandatory
  • Willingness and ability to travel within the community
  • Background in older people-related fields is not required
