About the role
Key responsibilities & impact- Manage AltaMed Managed Care participants assigned to the team
- Work closely with providers and nurses to coordinate services and advocate for participants
- Complete daily notes
- Conduct family conferences
- Assist with community referrals
- Maintain regular contact with participants
- Complete incident reporting
Requirements
What you’ll need- A bachelor’s or master’s degree in social work from an accredited university is required
- Minimum of 2 years of experience in case management and social advocacy required
- Experience working with the homeless population is preferred
- Managed Care Experience preferred
- Bilingual English/Spanish strongly preferred
