About the role
Key responsibilities & impact- Manage AltaMed Managed Care participants assigned to the team.
- Work closely with providers and nurses to coordinate services and advocate for participants.
- Complete daily notes.
- Conduct family conferences.
- Assist with community referrals.
- Maintain regular contact with participants.
- Complete incident reporting.
Requirements
What you’ll need- A bachelor’ or master’s degree in social work from an accredited university is required.
- Minimum of 2 years of experience in case management and social advocacy required.
- Experience working with the homeless population is preferred.
- Managed Care Experience preferred.
- Bilingual English/Spanish strongly preferred.
