About the role

Key responsibilities & impact
  • Perform in-home visits with patients
  • Travel to patients' homes in the Casper, Wyoming area
  • Complete point-of-care testing, including diabetic A1c collection, blood pressure checks, colorectal screening kits, and other testing required by STARS/HEDIS measures
  • Review collected data to confirm gaps in care are captured
  • Build positive, professional relationships with patients
  • Explain point-of-care testing and its reasoning to patients
  • Help improve patient care quality and reduce cost of care
  • Travel up to 80% of the time in a personal vehicle as part of an in-office/field position

Requirements

What you’ll need
  • High school diploma or GED equivalent required
  • Must have their own reliable vehicle to drive out in the field to patients homes
  • Must have valid state driver license and appropriate liability insurance
  • Certified Nurse Assistant (CNA) Certification or Medical Assistant (MA)
  • Current CPR required
  • 2+ years' experience in a healthcare environment
  • Ability to perform point-of-care testing such as urine sample collection, A1c checks, blood pressure checks
  • Familiar with medical terminology
  • Excellent written and verbal communication, relationship-building, and problem-solving skills
  • Highly organized and able to prioritize one's own workload
  • Strong customer service skills with exceptional attention to detail
  • Proficient with Microsoft Office
  • Previous experience using EMR systems would be an advantage
  • Must be eligible to work in the United States, as confirmed through E-Verify
  • Required to submit to a pre-employment drug test and background check
