About the role

Key responsibilities & impact
  • Provide physical therapy services in the UW Health Home Health department
  • Work a 100% FTE full-time schedule
  • Work day/evening shifts, Sunday through Saturday, between 0800 and 1700
  • Deliver outpatient physical therapy and direct patient care
  • Meet UW Health requirements for licensure eligibility before practicing as a therapist or assistant

Requirements

What you’ll need
  • Graduate of an accredited Physical Therapy program (or foreign equivalent as determined by the State of Illinois Department of Professional Regulation)
  • Current license from the State of Illinois
  • Basic Life Support/CPR Certification
  • Current, valid driver’s license issued by the individual’s state of residence
  • Acceptable evidence of licensure or authorization to test must be presented at the time of employment
  • Must provide documentation of licensure eligibility before working as a therapist or assistant
