About the role
Key responsibilities & impact- Collaborate with an interdisciplinary team of MDs, APPs, Social Workers, Schedulers, and RN Care Coordination nurses
- Provide care coordination, triage, and case management for patients enrolled in the Home-Based Primary Care program
- Communicate with patients and families via telephone or video communication
- Triage emergent issues
- Provide resources to clients and their families
- Initiate and facilitate consults and referrals
- Update care plans
- Coordinate care with rehabilitation and long-term-care facilities
- Care for homebound adult and geriatric patients
Requirements
What you’ll need- Graduate of school of nursing Required
- Bachelor's Degree in Nursing or higher Preferred
- 6 months RN experience Required
- Previous experience in adult acute care or ambulatory setting Preferred
- Previous case management experience Preferred
- Registration as a registered nurse in the state where employed or licensure in the state licensure compact upon hire Required
- CPR/BLS upon hire Required
- Case Management Certification Preferred
- Monday-Friday availability for 8- or 9-hour day shifts between 7:30 am and 5:30 pm
- Ability to work onsite at the Administrative Office Building on UW Health Court or the Northport Dr. Clinic in Madison, WI when required
