About the role
Key responsibilities & impact- Utilize clinical and community-based interventions to assist a diverse patient population with complex medical and psychosocial needs
- Address challenges and barriers affecting patient engagement
- Improve patients’ self-management of illness within a medical case management framework
- Work with patients over time to support adjustment and long-term health
- Participate in a dynamic, multidisciplinary HIV clinical care and prevention program funded by the national Ryan White HIV/AIDS program
- Opportunity to cofacilitate a support group
Requirements
What you’ll need- Master's Degree in Social Work required
- Certified Advanced Practice Social Worker (CAPSW) in the state of Wisconsin within 90 days required, or Certified Independent Social Worker (CISW) in the state of Wisconsin within 90 days required, or Licensed Clinical Social Worker (LCSW) in the state of Wisconsin within 90 days required
- Two years of prior hospital-based social work experience preferred
- Two years of prior hospital or community-based social work experience if supporting the HIV Clinic preferred
- Two years of prior hospital or clinic-based pediatric experience if supporting American Family Children’s Hospital (AFCH) preferred
- Full-time availability, 40 hours per week
- Primary availability Monday–Friday, 8:00 am–5:00 pm
