About the role
Key responsibilities & impact- Conduct comprehensive biopsychosocial assessments for psychiatric patients
- Maintain PA1/PS2 medication-only workflows and track pending PA2 requests
- Coordinate with prescribing clinicians to ensure timely reviews, service access, and continuity of care
- Provide crisis intervention, triage, de-escalation, and risk assessment when the Psychiatric Nurse Practitioner is unavailable
- Conduct periodic check-ins with medication-only patients
- Develop, implement, and update individualized, goal-oriented treatment plans
- Assist with clinical documentation and external requests, including FMLA forms
- Maintain accurate patient records in the EHR system
- Coordinate care, facilitate referrals, and address barriers to treatment engagement
- Participate in clinical supervision, case consultations, and professional development activities
- Identify workflow and follow-up improvement opportunities
- Perform additional clinical, care coordination, and administrative duties as assigned
- Support Athena’s psychiatry department as part of a multidisciplinary treatment team
Requirements
What you’ll need- Master's Degree from an accredited school in Social Work or other closely related field
- License as an LMSW or LCSW required
- Bilingual in English and Spanish is strongly preferred
- Passion for working with underserved communities
- High level of engagement, entrepreneurial, and interpersonal skills
- Previous experience using computerized appointment scheduling systems and/or electronic Medical Record systems
- Knowledge of NYS OMH Outpatient Clinic regulations, HIPAA regulations, and NYS mandated reporting requirements
- Demonstrates emotional intelligence in personal interactions
- Exceptional written and verbal communication skills
- Strong computer skills and proficiency with Word, Excel, G-Suite, and other software/applications
- Ability to hear and speak on the phone or computer throughout the day
- Ability to stand and sit for extended periods and write and answer emails
- Ability to carry objects no greater than 25 pounds when needed
