About the role

Key responsibilities & impact
  • Provide comprehensive care coordination for assigned patients
  • Assess patient plans of care and develop, implement, monitor, and document resource utilization and patient progress
  • Facilitate options and services to meet patients’ healthcare needs based on patient need and payer requirements
  • Manage patients across the healthcare continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes
  • Provide assessment, direct psychoeducation, and direct psychotherapy to individuals, families, and groups
  • Coordinate patient care from admission to discharge
  • Interact with physicians and clinical treatment team members to provide quality patient care
  • Lead process improvement activities for patient populations
  • Evaluate medical necessity and appropriateness of care
  • Establish collaborative relationships with physicians, payers, and healthcare team members
  • Collect and communicate timely information to payers and others for utilization and regulatory requirements
  • Collaboratively develop and manage interdisciplinary patient discharge plans
  • Communicate discharge plans across the continuum of care
  • Educate internal healthcare team members on case management and managed care concepts
  • Facilitate integration of case management and managed care concepts into daily practice
  • Float between adult and adolescent units and IOP as needed

Requirements

What you’ll need
  • Master’s degree in social work or related field or equivalent experience
  • Proficiency level typically achieved with 5 years clinical experience
  • Licensure in at least one of the following categories, as required by state law: social work, professional counseling, or marriage and family
  • Certification requires two years post graduate experience in a mental health setting or equivalent combination of education and documented supervised clinical experience
  • Board licensure in addictions as required by state for positions providing direct services to substance abuse program patients
  • Working knowledge of care management, hospital and community resources

  • Effective communication skills
  • Time management skills
  • Ability to work effectively in an interdisciplinary team format
  • Arizona Fingerprint Clearance Card required at hire and maintained during employment for applicable Arizona behavioral health roles serving children
  • Arizona Criminal History Affidavit required upon hire for applicable Arizona behavioral health roles
  • Colorado Fingerprint Clearance Card required at hire and maintained during employment for Banner McKee Senior Behavioral Health Inpatient Unit employees
  • State of Arizona Department of Public Safety Level One Fingerprint Clearance Card required at hire and maintained during employment for any Banner Staffing Behavioral Health role in Arizona
