About the role

Key responsibilities & impact
  • Lead approximately 25 Case Managers, Registered Nurses, Social Workers, and Transitional Care Associates
  • Provide managerial and clinical support to patients and staff
  • Oversee clinical, financial, and personnel management of the Care Coordination department
  • Provide and facilitate care coordination and utilization management through interdisciplinary patient care and discharge-plan activation
  • Recruit, interview, select, hire, train, evaluate, schedule, and direct staff workflow
  • Develop and implement department goals, objectives, budgets, and process-improvement activities
  • Support innovation and change management
  • Analyze data and healthcare trends to improve efficiency and patient outcomes
  • Facilitate communication among healthcare team members, patients, and families
  • Oversee patient satisfaction results and coach performance-improvement activities
  • Ensure orientation and expertise regarding protective services, guardianship, socioeconomic needs, homelessness, abuse, neglect, placement, and transitional-care options
  • Provide facility-based leadership for transitional care coordination, quality outcomes, and resource efficiency
  • Collaborate with nursing leadership, physicians, agencies, vendors, payers, community organizations, and regulatory bodies

Requirements

What you’ll need
  • RN candidates require a Bachelor’s of Science in Nursing or a related field
  • Current Registered Nurse (R.N.) license in the state worked (RN pathway)
  • Social Worker candidates require a Master’s degree in Social Work
  • Licensed Master Social Worker (LMSW) equivalent or Licensed Clinical Social Worker (LCSW), or an equivalent licensed-social-worker credential where applicable (Social Worker pathway)
  • Basic Life Support (BLS) certification required for acute care assignments
  • Minimum of two years’ experience providing care in an acute patient setting
  • Two years of related acute care patient management experience demonstrating care coordination knowledge
  • Knowledge and skills in the specialty area
  • Critical thinking, communication, influence, decision-making, analytical, and flexibility skills
  • Ability to practice skills according to department and professional standards and quality requirements
  • CCM (Certified Case Manager) or other advanced case management certification preferred
  • Additional related education and/or experience preferred
