About the role

Key responsibilities & impact

Evaluate patients' functional abilities and limitations and determine whether intervention is needed
Establish plans of care, treatment goals, discharge planning, and recommendations to referring providers
Implement individualized physical therapy treatment plans and provide skilled treatment
Assess treatment effectiveness and modify plans of care as indicated
Involve patients and families/caregivers in treatment and decision-making
Provide education to physicians, nurses, patients, families/caregivers, and interdisciplinary teams
Document evaluations, treatment techniques, patient progress, and reassessments within departmental time frames
Communicate patient information, evaluation and treatment results, and progress to care teams
Contact physicians for routine or difficult problem solving and defend treatment rationale
Adapt assignments to caseload, scheduling, and staffing needs
Assist in other patient care areas and accept responsibility for patient care needs
Supervise and delegate to PTAs, ATCs, therapy technicians, and students
Train and orient new personnel and assure delegated personnel competency
Attend required meetings and participate in departmental/facility process improvement
Meet department productivity standards
Interact with staff, physicians, patients, families, external healthcare institutions, community providers, agencies, and physician offices

Requirements

What you’ll need

Graduated from an APTA accredited school of Physical Therapy, or equivalent
Current Physical Therapy license within the State of practice
BLS certification required
Minimum commitment of 2 shifts a month
Completion of post-offer Occupational Health physical assessment, drug screen and background check, including employment, criminal and education checks
Ability to become proficient with computer software applications commonly used in an automated patient care environment
Additional related education and/or experience preferred
Employees working for Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of employment
