About the role
Key responsibilities & impact- Conduct high-volume telephonic outreach to health plan members
- Connect members with appropriate healthcare and community resources
- Document member interactions and interventions accurately and promptly in the EHR
- Coordinate referrals, home health care, appointments, outpatient services, benefits, and follow-up activities
- Review patient information, demographics, and healthcare data to identify changing needs and care gaps
- Perform follow-up calls to patients and providers regarding experiences and issue resolution
- Assist members in navigating the healthcare system and report health-status abnormalities or needs to licensed professionals
- Promote continuity of care by communicating patient status to caregivers and clinical teams
- Participate in departmental improvements, Banner initiatives, data collection, and project measurement
- Work reports, dashboards, and chart audits to close preventative and chronic-care gaps
- Collaborate with team members, vendors, community resources, and referral organizations
- Provide community resource information and perform related administrative or cross-coverage tasks
- Work under general supervision and consult supervisors on unusual situations
Requirements
What you’ll need- High school diploma/GED or equivalent working knowledge
- Three years of experience in healthcare, typically as a Nursing Assistant, Medical Assistant, Health Unit Coordinator, Patient Care Tech, or similar
- Current CNA, CHW, MA certification, or LPN license in state of practice may be required
- Understanding of medical terminology, clinical technology, healthcare processes, and EHR
- Effective communication skills
- Human relations skills
- Strong organizational and time management skills
- Flexibility in responding to multiple demands
- Bilingual ability preferred in some assignments
- Additional related education and/or experience preferred
- Must reside in the state of Arizona
