About the role
Key responsibilities & impact- Assess patients' physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs
- Review patient history, physical examinations, diagnostic and laboratory data, and report abnormal results
- Formulate, initiate, review, and revise individualized plans of care and discharge plans
- Provide direct patient care and coordinate, delegate, and supervise health care team activities
- Provide care based on physician orders, nursing plans of care, policies, standards, and regulatory requirements
- Evaluate patient and family responses to care and teaching, patient progress, and expected outcomes
- Document assessments, plans, interventions, evaluations, patient responses, and physician orders accurately and promptly
- Participate in research, shared leadership, education, orientation, precepting, mentoring, and quality improvement activities
- Interact with staff, physicians, patients, families, community providers, agencies, and clergy regarding patient care
- Work autonomously in patients' homes within an assigned East Valley geographic territory
Requirements
What you’ll need- Current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice
- BLS certification required
- Additional certification or continuing education may be required based on area of practice
- Clinical performance competencies appropriate to the work area must be maintained through annual validations
- Ability to operate a personal vehicle
- Valid driver's license
- Active and maintained auto insurance coverage
- Arizona Fingerprint Clearance Card required at the time of hire and throughout employment for Banner Home Care AZ employees
- Experience in the clinical area is desired
- Bachelor's degree preferred
- Professional certification preferred
- Additional related education and/or experience preferred
