About the role

Key responsibilities & impact
  • Provide inpatient and outpatient complex care coordination and oversight for Advanced Lung Disease patient populations
  • Serve as the point of contact and liaison for patients and caregivers
  • Provide extensive patient teaching and education
  • Monitor patient progress and plan-of-care adherence
  • Manage acute calls, patient needs, and symptoms of advanced therapies
  • Advocate for patients and intervene as necessary
  • Coordinate complex care with specialists, primary providers, physicians, and resources
  • Assess, address, remove, and prevent barriers to care
  • Assess plans of care and develop, implement, monitor, and document resource utilization and patient progress
  • Evaluate medical necessity and appropriateness of care
  • Assist referring physicians with scheduling initial tests and consultations
  • Coordinate disease care using an interdisciplinary holistic approach
  • Explain diagnostic processes, care strategies, recommended actions, disease processes, expected side effects, and community resources
  • Evaluate functional abilities and limitations, establish treatment goals, and implement plans of care
  • Initiate discharge planning and recommend modifications to referring orders
  • Support patients and families during difficult decision-making and coordinate end-of-life care
  • Support process improvement activities and participate in staff development
  • Establish collaborative relationships with physicians, payers, and healthcare team members
  • Collect and communicate information to fulfill utilization and regulatory requirements
  • Work independently within established procedures and confer with a supervisor on unusual situations

Requirements

What you’ll need
  • Bachelor's degree in nursing or related field
  • Current RN licensure in the state of practice
  • BLS certification required
  • 5 years clinical experience
  • Excellent organizational skills
  • Clinical knowledge regarding specialty care services
  • Knowledge of care coordination of services, legal and financial aspects of diagnostic services and health services in specialty area
  • Effective communication and writing skills
  • Good time management skills
  • Knowledge of word processing and database software applications
  • Ability to teach clinical and non-clinical personnel regarding care and diagnostic services
  • Understanding of process improvement
  • Employees working at Banner MD Anderson on the Banner University Medical Center Phoenix campus must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card at the time of hire and maintain the card for the duration of employment
  • Ability to stand, walk, bend, squat, reach, and stretch frequently
  • May be required to lift up to 75 pounds
  • Must use standard precautions due to threat of exposure to blood and body fluids
  • Ability to communicate effectively through reading, writing, and speaking in person or on telephone
  • May require periodic to frequent use of a personal computer
  • Off-site travel may be required
