  • Assess, develop, and implement patient care plans for a specific patient population
  • Integrate specialized knowledge into patient and staff education, consultation, and coaching

  • Interact with staff, physicians, patients, families, external health care contacts, community providers, agencies, and clergy
  • Synthesize and prioritize data from multiple sources to support patients and families responding to changes in health status
  • Work primarily at Banner Ironwood and float to Banner Goldfield as required

Requirements

What you’ll need
  • Bachelor’s degree in nursing, healthcare or related field
  • Current, valid RN license in state of practice, temporary RN license in state of practice, or compact RN licensure for current state of practice
  • BLS certification required
  • Current certification in specialty area of practice such as wound/ostomy care, diabetic care, or pain management
  • 2–3 years of experience in the clinical area of specialty
  • Strong knowledge of specialty area
  • Critical thinking skills to assess patient needs and opportunities for staff education
  • Effective verbal and written communication and education skills
  • Good organizational and facilitation capabilities
  • Strong leadership and mentoring skills
  • Employees working for Banner Home Care AZ must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of employment
