About the role
Key responsibilities & impact- Conduct comprehensive aged care needs assessments for older people within the Aged Care Planning Regional area of Victoria (VIC 3 – Loddon Mallee)
- Deliver high-quality, person-centred assessments supporting older Australians to access appropriate aged care services
- Conduct face-to-face assessments requiring travel within the Loddon Mallee region
- Conduct remote assessments via telehealth
- Provide clinical expertise and guidance to Non-Clinical Aged Care Needs Assessors
- Undertake additional functions such as delegation and triage
- Work as part of the My Aged Care Single Assessment Workforce
Requirements
What you’ll need- Full driver's licence
- Mobile phone
- Evidence of full immunisation record as part of the onboarding process if successful
- Cover letter and up-to-date resume
- Ability to conduct comprehensive aged care needs assessments
- Clinical expertise to guide Non-Clinical Aged Care Needs Assessors
- Ability to travel within the Loddon Mallee region
- Ability to undertake face-to-face and telehealth assessments
