About the role

Key responsibilities & impact
  • Conduct comprehensive assessments for older people within the Aged Care Planning Regional area of Victoria (VIC 3 – Loddon Mallee)
  • Deliver high-quality, person-centred assessments supporting older Australians in accessing appropriate aged care services
  • Conduct face-to-face needs assessments requiring travel within the Loddon Mallee
  • Conduct remote assessments via telehealth
  • Provide clinical expertise and guidance to Non-Clinical Aged Care Needs Assessors
  • Ensure assessments are accurate, holistic, and aligned with relevant aged care standards and policies
  • Undertake other functions such as delegation and triage

Requirements

What you’ll need
  • Health professional qualification sufficient for classification under the Allied Health or Nursing EBA
  • Full driver's license
  • Mobile phone
  • Evidence of full immunisation record as part of the onboarding process
  • Ability to conduct comprehensive aged care needs assessments
  • Ability to provide clinical expertise and guidance to Non-Clinical Aged Care Needs Assessors
  • Cover letter and up-to-date resume
