About the role
Key responsibilities & impact- Obtain documents and all relevant intake material
- Conduct individual intake evaluation and counseling sessions
- Share responsibility for education and other group programming activities
- Participate in client evaluations and case conferences
- Provide referrals for affected family members to self-help programs when applicable
- Assess patients’ continuing care needs
- Contact appropriate aftercare resources and arrange transfer or admission to rehabilitation facilities
- Liaise with self-help organizations
- Coordinate client transportation to the next level of care
Requirements
What you’ll need- Associates or Bachelor’s Degree in Human Services or related field preferred
- High School Diploma or Equivalent required
- A minimum of one year related experience and/or training in the area of addiction treatment
- Valid Driver’s License in good standing preferred
- Must be vaccinated against influenza (flu) as a condition of employment
