About the role

Key responsibilities & impact
  • Provide short-term school-based individual and group prevention services to youth at risk of substance misuse
  • Help youth develop coping skills, connect with community and school resources, and improve support networks
  • Provide preventative individual and/or group support to students under Senior Case Manager supervision
  • Coordinate with school support staff, faculty, administration, parents and caregivers
  • Assess academic and family needs and provide referrals to school and community support services
  • Provide triage support as needed
  • Facilitate prevention education activities addressing youth substance abuse, suicide prevention and mental health
  • Plan and facilitate prosocial student and family events
  • Provide outreach services in family homes and community locations
  • Attend weekly one-to-one supervision and other meetings
  • Maintain program tracking logs, paperwork, outcome measurements and satisfaction surveys
  • Represent the division, program and stakeholders at health fairs, focus groups, coalitions and special initiatives
  • Transport students as needed
  • Perform other duties as assigned

Requirements

What you’ll need
  • Bachelor Degree in Counseling, Psychology, Social Work or a related field; comparable experience will be considered
  • Experience and training working with youth, families and substance use issues
  • Working knowledge of computers
  • Ability to complete documentation electronically
  • Ability to complete written documentation timely and effectively
  • Valid driver's license, good driving record, and reliable vehicle
  • Must be able and willing to transport persons served
  • Travel and ability to meet persons served in a variety of outreach settings
  • Available at least one evening per week and occasional weekends
  • Flu vaccination required as a condition of employment
