About the role

Key responsibilities & impact
  • Serve as a leader in clinical pharmacy practice in anticoagulation and other clinical areas as determined by hospital needs
  • Provide comprehensive clinical and distributive medication management services
  • Use advanced clinical skills to provide evidence-based, safe, and cost-effective drug therapy
  • Serve as a drug information and educational resource for pharmacy staff, patients, residents/students, medical staff, nursing staff, and other healthcare professionals
  • Participate in developing clinical guidelines/protocols and new programs/services
  • Facilitate professional development activities
  • Practice pharmacy in accordance with Massachusetts regulations, departmental policies, and applicable federal requirements
  • Monitor patients in specific units/specialty services by collecting patient data, interpreting laboratory data, performing physical assessments, interviewing patients, and identifying therapy goals
  • Develop, administer, and improve ambulatory pharmacy programs and services
  • Independently develop drug use evaluations, ADR reporting, clinic development and management, and business plans
  • Initiate, adjust, monitor, and recommend drug therapy changes under protocol
  • Document clinical interventions in the electronic health record
  • Coordinate and interpret laboratory monitoring, including serum drug levels and electrolytes
  • Monitor and optimize patient outcomes, drug utilization, and compliance while detecting or minimizing ADRs and unnecessary drug use
  • Coordinate continuing education for medical and nursing staff and orient new pharmacy employees
  • Contribute to pharmacy newsletters, journal clubs, and professional in-services
  • Precept PGY1 and PGY2 residents and pharmacy students
  • Contribute to staff development and collaborate with clinical pharmacy specialists to improve pharmacy skills and workflows
  • Work Monday to Friday on the day shift

Requirements

What you’ll need
  • Bachelor's degree in Pharmacy required
  • Doctoral degree in Pharmacy preferred
  • Registration Massachusetts Pharmacist required
  • 1-3 years related work experience required
  • Completion of PGY1 accredited pharmacy residency or 3+ years of Clinical Pharmacy experience in hospital setting
  • Must obtain one or more certification (Certified Specialty Pharmacist (CSP), Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP)), within 18 months of hire
  • Understanding of insurance and adjudication processes
  • Experience with computer systems, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access
  • Pharmacy residency PGY2 specialty experience/fellowship or equivalent experience or 5+ years of clinical pharmacy experience preferred
  • Knowledge of complex/chronic diseases states and therapies used within specialty pharmacy preferred
  • Ability to communicate in English in written and verbal contexts
  • Ability to provide patient care across neonatal, youth, young adult, middle age, and elderly age groups
  • Ability to exert up to 20 pounds of force frequently and stand or move throughout the hospital campus
