About the role

Key responsibilities & impact
  • Provide compassionate personal care and homemaker services for hospice patients
  • Assist with personal hygiene and grooming, including bathing and mouth, skin, nail, and hair care
  • Assist patients with toilet facilities, including bed pans
  • Provide skin care and observe for pressure areas and sores
  • Assist patients in and out of bed using safe transfer techniques and lifting equipment
  • Assist patients with walking and mobility devices
  • Cue self-administered medications
  • Prepare meals and assist with feeding when required
  • Observe, report, and document patient status and care provided
  • Take and record temperature, pulse, and respirations as directed
  • Practice basic infection control procedures
  • Document patient functioning and changes for the hospice nurse case manager
  • Maintain a clean, safe, and healthy environment in patients' homes
  • Recognize emergencies and contact emergency personnel and supervisors when necessary
  • Treat patients and their property with respect

Requirements

What you’ll need
  • Valid MA Certified Nursing Assistant (CNA) certification OR Home Health Aide (HHA) certificate required
  • Must be vaccinated against influenza (flu) as a condition of employment
  • Ability to provide personal care and homemaker services according to the IDT plan of care
  • Ability to use safe transfer techniques, gait belts, and mechanical lifting equipment when applicable
  • Ability to assist patients with walkers, canes, and wheelchairs
  • Ability to observe, report, and document patient status and care provided
  • Ability to take and record vital signs
  • Knowledge of basic infection control procedures
  • Ability to recognize emergencies and contact emergency personnel and supervisor when necessary
  • Understanding of the physical, developmental, and emotional needs of served populations
