About the role
Key responsibilities & impact- Plan, organize, collaborate, and direct home care services.
- Coordinate patient care for assigned temporary caseload, including delegation and supervision of COTA staff, patient condition change visits, and completion of OASIS time points.
- Complete and document initial patient and family assessments to determine home care needs.
- Develop, initiate, and revise patient-specific plans of care.
- Initiate preventive and rehabilitative therapy procedures; administer medications and treatments as prescribed.
- Counsel patients and families regarding therapy and related needs.
- Provide health care instructions according to assessment and plan of care.
- Instruct, supervise, and evaluate home health aide care every two weeks as necessary.
- Manage episode or coordination of care with primary disciplines.
- Identify and implement discharge planning needs.
- Communicate with physicians regarding patient needs and changes in condition; obtain physician orders.
- Attend case conferences and team meetings.
- Coordinate care plans with COTAs, team disciplines, community providers, insurance case managers, elder services, and protective services.
Requirements
What you’ll need- Graduate of an accredited program in Occupational Therapy.
- Licensed, certified Occupational Therapist with current licensure to practice therapy in the state of Massachusetts
- Active American Health Association BLS
- Minimum 2 year’s experience preferred
- Oasis experience preferred
