About the role

Key responsibilities & impact
  • Serve as a subject matter resource regarding local addiction treatment and recovery support systems
  • Engage individuals in the ED seeking detoxification or substance use treatment and provide education on treatment and recovery options
  • Develop relationships with detoxification and treatment centers to streamline referrals and admissions
  • Identify treatment beds based on individual needs, preferences, and eligibility
  • Facilitate treatment-center admissions and coordinate required referral documentation
  • Track referrals and follow up with treatment centers and patients
  • Conduct post-discharge telephone outreach to assess treatment and recovery engagement
  • Connect individuals with Recovery Coaches, peer recovery support, harm reduction services, mutual aid groups, and community resources
  • Collaborate with the Addiction Consult Service Recovery Coach and healthcare teams to ensure continuity of support
  • Document patient interactions, referrals, outreach, and care coordination activities in Epic
  • Advocate for individuals while respecting their choices, goals, culture, and readiness for change
  • Maintain professional boundaries and confidentiality while utilizing personal recovery experience
  • Complete required organizational and role-specific training and maintain competency in addiction treatment, recovery, harm reduction, and community resource systems

Requirements

What you’ll need
  • High School diploma
  • Training or relevant work or lived experience in mental health and/or addiction/recovery preferred
  • Bachelor's Degree in psychology, social work, or related human services field strongly preferred
  • Candidates in their last semester of a Bachelor's degree program may be accepted under waiver
  • Experience delivering community-based or hospital-based services is a plus
  • Certificates, Licenses, Registrations, LADC/CADC preferred
  • Must be vaccinated against influenza as a condition of employment
