About the role
Key responsibilities & impact- Perform RN duties in an outpatient primary care or family medicine practice
- Provide direct, high-quality patient care within a team-based model
- Serve as a clinical resource for the medical practice
- Coordinate clinical management and care coordination for an outpatient population
- Collaborate with providers and community-based providers on preventive care, chronic disease management, transitions of care, ED visits, and inpatient discharges
- Provide healthcare education and support patient self-management goals
- Perform skilled telephonic triage, assessment, education, and care coordination
- Conduct Annual Wellness Visits and Chronic Care Management
- Manage INR and provide Coumadin teaching under standardized protocol
- Administer vaccines and medications according to provider orders and medication administration policies
- Perform wound care, point-of-care testing, examinations, treatments, and other nursing activities
- Document patient care in the Electronic Health Record
- Schedule tests and specialty-care referrals
- Monitor and follow up on abnormal test and imaging results
- Manage clinical supply and equipment inventory and ordering
- Collect, store, and process biologic specimens safely and accurately
- Provide oversight and delegation to MAs, Certified Nursing Assistants, and Nursing Students
- Educate and train practice staff and maintain clinical knowledge and mandatory training compliance
- Protect PHI and report adverse events and near misses to Risk Management
Requirements
What you’ll need- Graduate from an accredited Nursing Program required
- Registered Nurse license in Massachusetts required
- Certificate 1 Basic Life Support required
- CPR Certification
- 1–3 years related work experience required; new graduate RNs will be considered
- Strong interpersonal skills
- Strong patient communication and patient education skills
- Advanced skills with Microsoft applications, including Outlook, Word, Excel, PowerPoint or Access, and other web-based applications
- Ability to communicate clearly and effectively in written English
- Ability to comprehend and converse in English
- Ability to provide patient care to young adults, middle-aged adults, and elderly patients
- Ability to work with PHI and follow patient safety practices
- Ability to meet medium physical work requirements, including lifting up to 50 pounds occasionally and up to 20 pounds frequently
- Ability to work in environments with potential exposure to infectious diseases and bloodborne or airborne pathogens
