About the role

Key responsibilities & impact
  • Promote and deliver a holistic, client-centered approach to care aligned with Consumer Directed Care principles
  • Contribute to assessments, prevention, early detection, referrals, and health promotion activities
  • Deliver comprehensive, safe, effective, and evidence-based care aligned with current nursing care plans
  • Ensure compliance with relevant regulations, legislation, and organisational policies and procedures
  • Formulate, review, and participate in case conferences for care plans
  • Ensure care plans accurately reflect customer needs and support Aged Care Funding Instrument (ACFI) funding

Requirements

What you’ll need
  • Bachelor’s Degree required; Nursing, Healthcare is preferred
  • Any other related discipline or commensurate work experience considered
  • Minimal experience as a Registered Nurse in a hospital, clinical or residential setting, with expertise in patient care and medical procedures is preferred
  • Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA) Certification is prescribed
  • Strong knowledge of Clinical Assessment, Medication Administration & Monitoring, Person-Centered Practice and Quality Care Service Management skills
  • Excellent communication, adaptive thinking, planning and prioritisation, and cross-functional team working skills
  • Availability across Wednesday, Thursday and Friday from 8:00am - 4:00pm
