About the role
Key responsibilities & impact- Initiate face-to-face contact with eligible patients to describe the role, explain participation benefits, and begin screening
- Schedule and complete hospital, clinic, or community-based visits, screenings, care plans, follow-up visits, and phone calls
- Teach educational messages using culturally, linguistically, and educationally appropriate strategies
- Document activities, assessments, patient updates, and data in patient records, care management systems, and Epic
- Participate in community outreach, presentations, material development, and phone calls
- Work with patients and providers to set care goals and provide guidance
- Reinforce disease self-management education by connecting clients with community services and programs
- Present patients at case review meetings
- Consult with Patient Care Managers, clinical staff, behavioral health teams, and PCPs regarding complex patient situations
- Function within an interdisciplinary team and connect patients with needed resources
- Record and monitor participant progress toward goals
- Prepare reports and documents as needed
- Assist patients with organizing records, making and attending follow-up appointments, and filling prescriptions
- Help patients complete applications for Medical Assistance, Housing, and SNAP
- Provide advocacy, patient education, and warm handoffs to community- and hospital-based programs
- Help patients overcome barriers involving healthcare, behavioral health, financial assistance, childcare, housing, utilities, food, transportation, and other community resources
- Coordinate with community-based long-term services and supports
- Provide intensive home and community-based outreach, motivational interviewing, goal setting, resource connection, and appointment accompaniment
- Visit patients in hospital and ER settings to facilitate transitions of care
- Establish culturally appropriate, trusting relationships with patients and families
- Participate in designated training activities and scheduled supervision/program meetings
- Develop and maintain relationships with the community and community resources
- Do not provide hands-on care or home health services
Requirements
What you’ll need- HS Diploma with community experiences or Bachelor’s degree
- Driver’s license required
- (CCHW) Community health worker certification required
- Minimum of 2 years prior healthcare, public health, or community-based experience in community setting
- Basic knowledge of healthcare system
- Outstanding interpersonal skills to interact with families and patients
- Interest in community health and outreach
- Exceptional organizational skills; ability to multi-task and work independently and as part of a team
- Demonstrated oral and written English communication skills
- Fluency in Haitian Creole or Spanish preferable
- Understanding of how language, culture and socioeconomic circumstances affect health
- Desire to work with diverse, multi-cultural and multi-lingual populations
- Proficiency with Microsoft Office applications (MS Word, Excel, Access, Outlook) and web browsers
- Proficiency with data entry and data tracking
