About the role

Key responsibilities & impact
  • Receive phone calls from hospice patients and families
  • Assess patient and family needs and coordinate needed care over the phone
  • Schedule home visits as needed
  • Manage patients’ pain and symptoms pursuant to physicians’ orders
  • Obtain changes in orders as needed
  • Update patient care plans and medication lists
  • Assess needs for home visits and contact appropriate clinicians to schedule visits
  • Notify patients and families of expected visit times
  • Collaborate with transdisciplinary team members
  • Facilitate communication among the transdisciplinary team, Hospice, and the patient’s Attending Physician

Requirements

What you’ll need
  • Minimum of three (3) years of experience as a triage/telephonic nurse within the last five (5) years OR one (1) year of RN experience in hospice or palliative care
  • California RN license
  • CHPN certification, preferred
  • Compliance with CDPH vaccine requirements for healthcare personnel
