About the role

Key responsibilities & impact

Conduct outbound telephonic patient outreach and provide one-on-one coaching and support to patients managing chronic conditions.
Help patients execute personalized care plans and achieve goals through education and coaching on behavior modification, nutrition, physical activity, and self-management.
Use motivational interviewing and SMART goals.
Conduct comprehensive assessments of patients’ health status, lifestyle behaviors, nutritional habits, and readiness to change.
Educate patients on disease management, medication adherence, symptom recognition, and prevention strategies.
Execute care delivery tasks under appropriate clinical supervision and established protocols.
Monitor patient progress, treatment-plan adherence, and health outcomes through regular check-ins and remote monitoring tools.
Document patient interactions, outcomes, and follow-up plans in the Cadence platform.
Identify barriers to care and connect patients with resources or internal support pathways.
Collaborate with clinicians and patient support teams using Cadence Platform, Google Workspace, and Slack.
Contribute feedback to improve workflows.

Requirements

What you’ll need

An active compact multi-state LPN/LVN license in the state where you currently reside is required.
At least 5 years of clinical experience as a Licensed Practical Nurse is required.
Remote work readiness (including stable high-speed WiFi) is required.
An active CA state LVN license is required.
Fluency in Spanish is preferred.
Experience providing patient education and support to individuals managing chronic conditions is preferred.
Fluency with AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms) and the ability to speak credibly with providers and patients about where AI supports care delivery and where human judgment remains non-negotiable.
Availability to work at least three days per week during standard business hours of either 8:00 AM to 5:00 PM or 9:00 AM to 6:00 PM Pacific Time.
Availability to work Saturdays is required.
Comfortable using Cadence Platform, Google Workspace, Slack, and AI-assisted clinical tools.
Stable high-speed WiFi for remote work.
