About the role

Key responsibilities & impact
  • Provide direct supervision, training, and consultation to a team of four to five Supervisors
  • Provide direct and indirect supervision to Home Care Aides when needed
  • Ensure services comply with best-practice standards and Illinois Department on Aging Community Care Program requirements
  • Assist in directing, monitoring, and evaluating day-to-day operations
  • Review monthly participant, service, and staff reports and recommend improvements to the Director
  • Utilize time and labor systems, including Medsys EVV, Telephony, and Ultipro
  • Maintain accurate participant, service, HCA, and personnel files
  • Monitor supervisory and direct-service staff assignments, service delivery, participant information, and personnel processes
  • Ensure timely submission of Case Action Notices and participant database updates
  • Coordinate new participant referrals and assignments of Supervisors and Home Care Aides
  • Monitor Home Care Aide schedules, assignments, participant calls, and quarterly home visits
  • Conduct home visits and document supervisory contacts when needed
  • Monitor plans of care, service provision, and staff performance
  • Provide participant information, referrals, and linkage to community services
  • Provide orientation, training, supervision, consultation, and performance-management support to assigned staff
  • Attend staff meetings, in-services, and ongoing training
  • Participate in hiring interviews and regular supervision with the Senior Services Manager
  • Support Supervisor/HCA meetings and foster collaborative working relationships
  • Maintain knowledge of IDOA policies, agency policies, mandated reporting, incident reporting, and quality-improvement processes
  • Approve timesheets and ensure compliance with agency and funder guidelines
  • Maintain relationships with IDOA/CCP Care Coordination and Managed Care Organization units
  • Engage in community outreach and promotion of the Home Care Services program

Requirements

What you’ll need
  • High school diploma or GED required
  • Minimum of four (4) years of proven supervisory experience in home care services or a similar older adult service setting
  • Bachelor’s degree in Social Work, health or human services, or related field preferred
  • Must successfully complete supervisory training provided by IDOA or its designee
  • Bilingual in Spanish and English required
  • Computer literacy required
  • Proficiency in Microsoft Office applications including Outlook, Word, Publisher, and Excel
  • Ability to learn and effectively utilize Time & Labor Software, including Medsys EVV, Telephony, and Ultipro systems
  • High school-level math skills required; ability to prepare and analyze programmatic statistical reports
  • Ability to work independently and collaboratively with diverse populations
  • Effective communication, supervisory, and relationship-building skills
  • Exceptional time management and organizational skills
  • Personal cellphone required and ability to respond during working and non-working hours
  • Access to a car, valid Illinois Driver’s License, and vehicle insurance required
  • Ability to travel throughout the Chicagoland area and to other locations throughout the city or state
  • Subject to drug and alcohol screening, aptitude and skills testing, and fingerprinting background check
  • Must be able to lift and carry up to 25 lbs
