About the role

Key responsibilities & impact
  • Facilitate and coordinate regular checkpoints with representatives of designated Accounts
  • Maintain oversight of clinical files through collaborative contacts between clinicians and Account representatives
  • Provide effective referral review in collaboration with Accounts
  • Work with the National Manager on challenging files and requests outside typical service delivery pathways
  • Monitor Key Performance Indicators through communication with the Business Services team and internal escalation methods
  • Respect and apply rehabilitation care models and review their application by clinical teams
  • Ensure programs and models align with referral goals, typically with a strong vocational focus
  • Ensure accessibility to service, assessment and treatment services, documentation, reporting, communication and case management meets or exceeds standards
  • Review scheduling practices and service delivery for consistency in achieving expected clinical outcomes
  • Collaborate to resolve execution gaps and identify opportunities for team and program/service improvements
  • Collaborate with other Account Managers to innovate service delivery and achieve strong clinical outcomes, stakeholder experience, duration of care and durable outcomes

Requirements

What you’ll need
  • A bachelor's degree in a related discipline
  • 3 or more years of experience in a clinical or similar disability coordination role (e.g., Case Manager, Rehabilitation Coordinator)
  • Experience in the health, rehabilitation or vocational sectors is required
  • Experience in disability insurance sector, either in an insurer or employer role
  • Mental health experience is an asset
  • Ability to deliver timely, professional and responsive service to clients
  • Comfortable facilitating and negotiating care planning
  • Must be located within Canada
