About the role

Key responsibilities & impact

Coordinate timely, safe, and effective remote patient care using technical, organizational, and critical thinking skills
Coordinate patient care activities in a hybrid role spanning the Virtual Care Command Center and patients’ homes
Screen and triage patients
Manage a multidisciplinary team to ensure safe, timely, and quality care
Develop collaborative workflows, policies, protocols, and guidelines using virtual care team and service provider network resources
Assign patient-care activities while maintaining accountability for outcomes and nursing-care delivery
Collaborate with bedside staff, physicians, and interdisciplinary teams to coordinate care and support clinical decision-making
Facilitate admission and discharge processes, including patient intake, care coordination, and patient education
Educate patients and families about care plans, treatments, medications, and discharge instructions
Work closely with customers, physicians, and leaders

Requirements

What you’ll need

Graduate of an accredited nursing program required
Bachelor Degree of Science in Nursing or above preferred
CA State RN license
American Red Cross (ARC) or American Heart Association (AHA) Advanced Cardiovascular Life Support (ACLS) required; expiration date must be more than 60 days from hire date
ANCC Specialty Certification; one year to obtain for external hires
ANCC Specialty Certification required upon hire for Per Diem
3 years acute care nursing experience in a specialty area, including 1 year of virtual care nursing and/or nursing informatics experience, required
3 years acute care nursing experience in critical care preferred
1 year of charge or supervisory experience preferred
1 year of experience as Clinical Nurse III at CSMC preferred if internal
