About the role
Key responsibilities & impact- Ensure physician services are provided to the Patient Access Department to facilitate timely access to hospice care
- Provide physician direction and education to admissions personnel
- Assist in obtaining additional information to establish patient eligibility
- Complete regulatory hospice physician documentation for each admission
- Collaborate with the Director of Patient Access Services and Patient Access/Admissions Department to maximize access to hospice care
- Review, confirm, and evaluate patient information from medical records, referring or primary physicians, and Patient Access/Admissions Nurses
- Determine initial and ongoing hospice eligibility based on prognostic indicators, Local Coverage Determinations, coexisting conditions, and disease progression
- Provide verbal certification of hospice eligibility
- Complete written certification and clinical narratives for eligible admitted patients
- Order medications promptly to promote comfort for newly admitted patients
- Evaluate the admissions process and recommend efficiency and accuracy improvements
- Educate Patient Access/Admissions and Professional Relations Departments on eligibility information and regulatory changes
- Determine related or unrelated medications, treatments, and hospitalizations and document them in the EMR
- Assist in reviewing hospital patients for appropriate levels of care and participate in hospital interdisciplinary groups
- Represent the Company professionally and comply with applicable regulations, policies, health and safety practices, confidentiality requirements, and quality improvement activities
Requirements
What you’ll need- MD or DO from an accredited medical school
- Licensed to practice medicine in the State of Florida, with willingness to obtain additional states as required
- Current DEA number for Florida and additional requested states
- State Controlled Substances Registration in Maryland and the District of Columbia
- Current Medicare and Medicaid Numbers
- Active BLS certification from the American Heart Association or the Red Cross, with in person training
- Minimum of five (5) years’ experience or Fellowship
- Knowledge and experience in palliative medical practice and symptom management
- Basic computer skills including email and word processing (Microsoft Outlook and Word or equivalent)
- Must be able to physically access any home in the Organization’s service area
- Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
- Eligible for malpractice coverage under Chapters’ policy terms
- Board eligible or board certified in a recognized field of the American Board of Medical Specialties preferred
- Certification by the American Board of Medical Specialties in Hospice and Palliative Medicine preferred
- Hospice and Palliative Medicine ABMS or HMDCB certification preferred if not already certified
- Consent to drug and/or alcohol testing after a conditional offer and ongoing compliance with the Drug-Free Workplace Policy
- Florida Care Provider Background Screening Clearinghouse eligibility after a conditional offer and ongoing eligibility
