About the role

Key responsibilities & impact
  • Assess and identify patient and family needs using the nursing process
  • Coordinate individualized Plans of Care with the Interdisciplinary Group (IDG)
  • Provide palliative, supportive, and direct care to patients and families
  • Complete clinical admissions for company services, including explaining services, addressing concerns, educating families, completing consents and assessments, initiating Plans of Care, and obtaining physician orders
  • Initiate skilled nursing interventions to prevent complications, alleviate symptoms, and maximize comfort
  • Complete documentation according to company policy
  • Represent the company at assigned facilities and facilitate referrals across service lines
  • Coordinate with Admissions for timely follow-up and assignment transitions
  • Provide timely clinical communication using SBAR
  • Discuss potential needs with after-hours staff
  • Obtain consents, arrange safe discharges, and obtain medical records for referrals
  • Build relationships with case managers, physicians, and facility contacts
  • Collaborate with Professional Relations Representatives to educate stakeholders about available services
  • Provide care for GIP patients in hospitals, including daily visits, clinical management, documentation, and discharge
  • Participate in IDG evaluations of hospice referrals and admissions for appropriate level of care
  • Educate patients, families, caregivers, and health professionals about disease processes, palliative interventions, caregiving, dying, and safety
  • Report patient condition changes to IDG members and other health professionals
  • Attend patient deaths, prepare bodies for transport, and oversee medication disposal
  • Provide bereavement interventions with psychosocial staff
  • Present oral and written reports to the IDG
  • Supervise care provided by LPNs, Hospice Aides, and contract nurses
  • Report to a company-designated location during emergencies when required, potentially ahead of schedule or overnight
  • Perform other duties as assigned

Requirements

What you’ll need
  • Current license as RN in the state where the employee will be working
  • Active BLS for healthcare professionals from the American Heart Association or Red Cross
  • Minimum one (1) year nursing experience in hospice or hospital experience preferred
  • Previous experience working with an EMR/EHR system
  • Valid driver’s license and automobile insurance per Company policy
  • Reliable transportation to meet visit schedule
  • Ability to effectively communicate in English, verbally and in writing
  • Flexible schedule to meet the needs of Hospice House patients
  • Ability to manage emotional stress when caring for terminally ill patients and families
  • Ability to access patient rooms, Hospice House/Hospital/ALF areas, and homes within the service area
  • Ability to perform essential physical job functions, including lifting, bending, pushing/pulling, and kneeling, with or without reasonable accommodation
  • Employees working at PACE must have certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs
  • Satisfactory completion of competency requirements
  • Consent to drug and/or alcohol testing after a conditional offer and ongoing compliance with the Drug-Free Workplace Policy
  • Eligibility verification through the Florida Care Provider Background Screening Clearinghouse after a conditional offer and ongoing eligibility
