About the role
Key responsibilities & impact- Assess and identify patient/family needs using the nursing process
- Coordinate individualized Plans of Care with the Interdisciplinary Team (IDT)
- Provide direct clinical, palliative, supportive, psychosocial, spiritual, cultural, physical, and biological care
- Provide reassurance and support to patients and families by phone and coordinate in-person visits when needed
- Utilize support and expert resources to resolve complex situations
- Document patient/family contacts and care in the EMR
- Complete initial and semi-annual assessments for Company services
- Explain services and address patient/family questions and concerns
- Provide information to physicians and IDT members and initiate Plans of Care
- Initiate skilled nursing interventions to prevent complications, alleviate symptoms, and maximize comfort
- Obtain physician orders and complete documentation according to Company policy
- Represent the Company at assigned facilities and facilitate referrals across service lines
- Communicate clinical information using SBAR and participate in daily IDT collaboration meetings
- Develop relationships with case managers, physicians, facilities, patients, families, and referral sources
- Conduct home visits to assess safety, medication compliance, nutritional compliance, DME compliance, and ability to live safely in the community
- Report patient condition changes to the IDT and other health professionals
- Participate in evaluating hospice referrals/admissions for level-of-care appropriateness
- Educate patients, families, caregivers, and health professionals about disease processes, palliative interventions, caregiving, dying processes, and safety
- Supervise care provided by Community Health Workers and Home Health Aides as requested
- Report to a company-designated location and potentially stay overnight during emergencies
- Perform other duties as assigned
Requirements
What you’ll need- Current license as RN in the state where the employee will be working
- Minimum of one (1) year nursing experience in hospice or hospital experience preferred
- Employees working at PACE must complete Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs
- Previous experience working with an EMR/EHR system
- Valid driver’s license and automobile insurance per Company policy
- Reliable transportation to meet visit schedule
- Ability to use equipment with visual and auditory mechanisms
- Ability to effectively communicate in English (verbal and written)
- Ability to visit participants in their homes to perform assessments
- Ability to perform essential job functions and physical requirements, including lifting patients and/or equipment, bending, pushing/pulling, and kneeling, with or without reasonable accommodation
- Active BLS for healthcare professionals from the American Heart Association or Red Cross
- Satisfactorily complete competency requirements for this position
- Consent to drug and/or alcohol testing after a conditional offer of employment
- Ongoing compliance with the Drug-Free Workplace Policy
- Eligibility through the Florida Care Provider Background Screening Clearinghouse for Florida affiliates
